What are acceptable receipts for daycare expenses?
I'll be submitting all of my daycare expenses at once; what can I use for acceptable receipts? Also, I have 2 children in daycare do I list both of their ages in the "Who Service was for" section? Finally, because I paid per month do I put the beginning of each month as the "Date of Service"? Thanks for any help with this.

Hi Aleta, this link provides an example Daycare receipt that can be used or you can also just get the signature of your daycare provider on our claim form (under the "forms" section of our website at www.24hourflex.com).
You do not need to list either child's age in the "Who Service was for section".
Putting the beginning of each month as the "Date of Service" is perfect. Please let us know if you have any other questions.

The receipt must have the following on it to be an eligible receipt:
1. childs name
2. Day Care provider
3. The Dates of Service
4. The Tax payer ID number or Social Security number for Tax purposes.
5. The total amount you ar claiming -
